Allegation: Facility staff is restricting resident from having visitors
Finding: Substantiated
During investigation on 8/4/2026, LPA interviewed witness one (W1). W1 stated that they gave authorization for W1 to bring R1 back to the facility. W2 stated that when they called the facility to let them know that W1 was bringing R1, W3 stated over the phone that W1 was not allowed to go into R1’s room. On 8/25/2026, LPA interviewed W3 that stated that on 3/27/2026, they were made aware that R1 was coming back to the facility and W3 called S1. W3 stated that they were told by S1 that W1 could not go into R1's apartment. W1, W2 and W3 stated that W1 was denied access into R1’s apartment from W3.
Based on LPA's information obtained during investigation, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, are being cited on the attached LIC9099D.
