On 08/27/2026 at 12:00 PM, Licensing Program Analyst (LPA) David Doidge arrived unannounced to conduct an initial 10-day complaint investigation (and to deliver findings) regarding the allegations above.
LPA met with Executive Director Robert Alverado and explained the purpose of the visit.

The department received a complaint alleging that the facility staff did not safeguard a resident’s personal belongings. R1 alleges that three rings were stolen and R1 believes the perpetrators are staff. R1, interviewed at 12:50 PM in R1’s room, informed LPA that initially one ring went missing and R1 suspected two staff members with access to R1’s room worked together to take the ring. R1 stated a maintenance member came in to unclog the bathroom sink, and later when R1 went to put on the ring, R1 could not find it. R1 called the police and informed the facility of the missing ring. Since the initial ring went missing, R1 alleged that two more were taken. R1 showed LPA two of the three rings have been found in a drawer in R1’s room. One ring remains missing. R1 stated R1 no longer feels the ring was stolen and now thinks it could just be missing in R1’s room. LPA found R1’s story of events inconsistent.

LPA spoke with W1 at 8:58 AM, the same day. W1 stated R1 had previously called W1 to inform W1 that the rings were found and now more investigation was needed. W1 did not know if that meant all three were found.

LPA interviewed S1 at 12:05 PM. S1 informed LPA that R1 had initially informed S1 that one ring was missing and accused a maintenance member of the theft. Police were called and investigated. S1 informed LPA that S1 found R1’s story of events inconsistent and hard to follow.

Based on interviews conducted the allegation that staff did not safeguard residents personal belongings is UNSUBSTANTIATED.

The department was also notified that staff did not ensure resident did an inventory list at admission. During interview with W1, W1 informed LPA that R1 informed W1 that R1 had not been given an inventory list either at admission or since moving in. During LPA’s interview with R1, R1 stated R1 did not remember signing an inventory list upon admission. S1 provided LPA with R1’s signed inventory list dated May 19th, 2026. LPA showed R1 the inventory list. R1 confirmed it was R1’s signature but did not remember what it was for. The inventory list did not have any items listed on it.

Based on interviews conducted and the signed inventory list, the allegation of staff did not ensure resident did an inventory list at admission is UNSUBSTANTIATED.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.

No deficiencies were cited during this inspection.

Exit interview conducted and a copy of this report provided.
