On 07/10/2026 Licensing Program Analyst (LPA) M. Garza arrived at the facility for an unannounced complaint visit to deliver findings. LPA met with Administrator, Karely Alcantar, explained reason for visit and was permitted entry into the facility.

During complaint LPA completed interviews, requested and reviewed documenation and toured the facility inside and out. Review of records disclosed that R1's admission agreement stated independent living residents could purchase meals for an additional cost. Review of R1's medical assessment dated 03/30/26 indicated that R1 has a need for a special diet. Previous medical assessment dated 06/18/2009 did not specify this need. During the period of 06/2009 through 03/2026, R1 was instructed by staff to provide an updated medical assessment for the special diet accommodation. Interviews conducted indicated that the facility provides accommodations for special diets to those with medically documented conditions. Although the allegation may or may not have occurred, it does not meet the prepondance of eviidence standard per California Code of Regulations, Title 22. The allegation listed above is UNSUBSTANTIATED. No deficiencies cited during todays visit.

Exit interview completed with Administrator, Karely. A copy of this report provided.
