At approximately 9:40AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to deliver findings from an investigation into the above allegations. LPA met with Administrator Ohshinnah Savage. During the course of this investigation, LPA conducted interviews and reviewed records. Based on interviews conducted, LPA did not find evidence supporting the allegations. Residents appeared to be clean and dressed. Meals appeared to be prepared in a safe and healthful manner and the amounts at each meal appeared to be generous. Snacks were available to residents throughout the day. The Licensee has two dogs, who were present at each visit. The dogs were well behaved and stayed with the Licensee wherever she went. LPA did not observe the dogs jump on residents or get in the way while residents were walking. LPA reviewed resident care plans and observed the facility staff are aware of each residents special needs and work with each resident to meet them. LPA observed that while staff are with residents they spoke in English so residents could understand.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated.
