32 | On 04/03/2026, LPA Chochian conducted the initial complaint visit. LPA met with Resident Care Coordinator, Tina Hernandez, Assistant Administrator, Alexander Solorio and Executive Director Rose Anguiano. During the initial visit, a physical plant tour was conducted at approximately 10 a.m. During the tour, LPA observed the common areas, and random resident rooms. Additionally, LPA met with and interviewed three (3) residents; additional residents were interviewed in the facility office. Total of twelve (12) residents were interviewed from approximately 10:45 a.m. - 12:15 p.m. LPA also conducted interviews with six (6) staff from approximately 12:30 p.m. - 1:30 p.m. Furthermore, records relevant to the investigation were obtained and copies were forwarded to the Department’s Investigation Branch.
On 04/13/2026, between the hours of 12:56 p.m. – 5 p.m., the Department conducted interviews with Resident #2 (R2) and Resident #3 (R3), and staff. Additional interviews were conducted on 04/22/2026 with staff and Resident #1 (R1). The Department also conducted interview with a credible witness on 4/16/2026 and reviewed facility records relevant to the investigation.
The investigation revealed R1 reported to Adult Protective Services (APS) that they were dating R2 and suspects that R3 was perusing R2 and providing R2 with drugs. R1 was concerned with R2 because they suspected that R2 and R3 were in a bad relationship. R2 denied any drug activity at the facility and denied ever being in a romantic relationship with either of the residents. It was confirmed that R1 did not witness any drug activity at the facility. Records reviewed documented the alleged illegal drug activity were thoroughly investigated by the staff and addressed by APS and Law Enforcement and confirmed there was no evidence of R2 or R3 involved in any drug related activity. In addition, R1 confirmed that “they did not see the drugs.”
Furthermore, information gathered during the course of the investigation revealed that residents have not seen any person use or sell illegal drugs in the facility. However, some residents may seem like they are under the influence yet, it is hard to determine if it is related to drugs or medical issues. The facility’s population consists of residents who are verbal and aware of their surroundings and would not hesitate to make such allegations known if they witnessed any drug activity in the facility. Staff interviewed denied the allegation and stated that they have no knowledge of any resident engaged in illegal drug activity at the facility. Administrators also stated that she has not heard any concerns related to drug use in the past few months. (Continue to LIC9099c) |
