13 | At approximately 9:45 a.m. on 03/05/26 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced complaint visit. LPA met with staff and disclosed the reason for the visit.
Regarding the allegation "Staff did not keep resident's records in a secure location" it was alleged the records of Resident #1 (R1) were not kept confidential and kept on a bookshelf. To investigate the allegation, LPA toured the facility inside and out at 9:50 a.m. today, interviewed staff and residents between 10:00 a.m. and 12:00 p.m., and conducted a record review of pertinent records, including but not limited to an admission agreement, medical assessment, and care plan at 11:15 a.m.
At approximately 10:10 a.m. today, LPA observed a locked file cabinet with confidential resident and staff files. Near the locked cabinet was an accessible shelf with medication administration records (MARs) and two (02) hospice files belonging to residents. |
