A review of hospice records states resident began receiving services May 22, 2024. Wound evaluation dated May 26, 2025 states weekly debridement to promote healing is initiated, clinical assessment left gluteal and left gluteal crease buttock pressure ulcer Stage 3 and that wound is unrelated to the hospice terminal illness diagnosis. A review of facility log states R1 was sent to the hospital June 10, 2026. During interviews, Lopez stated R1 was receiving hospice services in the facility when suddenly the agency closed May 15, 2026 without warning. We felt we could continue providing the wound care while searching for a new hospice agency but we could not find one in time so we had to send resident to the hospital.

Based on interviews, there is enough information to verify the allegation. Therefore, the allegation is SUBSTANTIATED at this time.

Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 9099-D):

No health and safety hazards noted during the visit.

Exit interview conducted and a copy of the report was issued.

A review of facility log states R1 was sent to the hospital June 10, 2026. A review of facility Personnel Report shows there are at least two (2) staff available day and night. During interviews, Lopez stated were all following the instructions provided by the hospice agency and did not expect them to close their business and we are not the cause of the wound. Lopez stated once they were all out of supplies to properly care for wounds, they decided to admit R1 to hospital to get the necessary care. Lopez stated wound supplies are expensive. S2 stated when the hospice nurses were not available to care for the wound, the caregivers are the ones cleaning the surrounding area and treating the wound. All staff stated they check on residents every two (2) hours. During interviews, all residents stated they are checked on often and do not feel neglected.

Based on interviews, there is not enough information to verify the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time.

No health and safety hazards noted during the visit.

Exit interview conducted and a copy of the report was issued.
