from the facility because the resident wanted to utilize a different hospice care provider than the hospice care provider that was preferred by the facility. Interviews with four (4) out of four (4) staff members did not reveal that any resident was evicted for not utilizing a particular hospice care provider. Four (4) out of four (4) staff members stated that there had been no residents evicted from the facility this year and that residents can choose whichever hospice service provider they prefer without being evicted. Licensee and Administrator stated that families who ask for assistance in finding a hospice care provider will be provided with names of agencies that have served residents at the facility in the past but the facility clearly communicates to the family that the resident can choose any hospice provider that the resident wants. Interviews with four(4) of four (4) residents did not reveal that staff unlawfully evicted a resident in care, Two (2) of four (4) residents stated they were not aware of any resident that had been evicted. Two (2) of four (4) residents stated residents had no knowledge about the eviction process. Interviews with four (4) out of four (4) residents revealed that the residents were not aware about hospice services or about which hospice agencies provide services at the facility. Interviews with three (3) out of four (4) family members of residents revealed that the facility does not evict residents for not utilizing a particular hospice service and that residents are free to choose whichever hospice service the resident prefers. One (1) out of four (4) family members stated that the facility will only allow residents to utilize a particular hospice service and not a hospice service of the resident's own choosing. Three family members of residents stated that the facility offers names of hospice providers that have serviced the facility in the past but did not communicate that these providers are the only providers permitted at the facility. Three family members of residents stated residents will not be evicted from the facility for not choosing a certain hospice provider. One family member of a resident stated that the facility did not evict anyone for not using a particular hospice agency but clearly communicated that the facility only wanted residents receiving hospice care from a particular agency. LPA' s record review did not show that any residents were evicted from the facility in 2026.LPA's record review revealed that one agency provided hospice care to four out of five residents receiving hospice services at the facility. The investigation did not reveal that staff unlawfully evicted a resident in care.
Based on interviews with staff, residents, residents' family members and a review of facility records there was not enough supportive evidence to concur with the reported allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
An exit interview was conducted with Administrator Hyun Joo Watanabe and a copy of this Licensing report was provided.
