32 | Allegation: Staff do not communicate effectively. It was reported that some staff members possessed virtually no English-speaking skills. The communication gap prevented them from conveying R1’s medical needs.
During interviews, LPA Comer noted that Administrator and assistant administrator are both fluent in English. According to the Administrator, S1 was not fluent in English. However, she was able to communicate with the residents, as well as with any medical and/or emergency personnel, as needed. Facility has no residents since October 2025. No other residents were available to be interviewed.
Based on overall investigation, there is no sufficient information to verify the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time
Allegation: Physical plant is not properly maintained. It was alleged that lack of stable staffing resulted in a staff member quitting mid-shift, which led to an un-monitored bathroom flood, creating a hazardous environment. Interviews revealed that between February 2025 and July 2025, the facility had 2 residents, and between July 2025 and September 2025, there was only one (1) resident present. The Administrator and assistant administrator denied any disrepair in the bathroom. Additionally, Admin and assistant Admin indicated they relieve each other to make sure that one of them is always present in the facility. Facility has two (2) bathrooms and they state that both were always in working condition. The Administrator stated that R1 was bed bound due to being overweight. R1 was not using the bathroom and instead using a commode for their toileting needs. During investigation LPA Comer observed the bathrooms to be clean and in working order.
Based on interviews, inspection and observation, there is no sufficient information to verify the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time.
No citations were issued, exit interview was conducted and a copy of report was issued to the Administrator. |
