Regarding the allegation: Unlawful Eviction. It is alleged that the facility unlawfully evicted resident #1 (R1) with no notice. During LPA’s interview with R1, R1 confirmed that they received their thirty (30) day eviction notice signed and delivered by the previous administration. LPA asked do you remember the day you received it and R1 stated, “yes, January 22, 2024, which confirms the Eviction Notice LPA reviewed and received. LPA asked R1 did you agree with it and R1 stated, “I did not want to return to the above facility and; in addition, they would have just given me an unlawful detainer and force me to leave.” During LPA’s record review of R1, it was confirmed that R1 was given an eviction notice on January 22, 2024, and several Unusual Incident/Injury Reports were provided to Community Care Licensing Department to support the eviction notice along with the Los Angeles County Department of Mental Health paperwork confirming that R1 had a grave disability/5150 hold dated 01/01/24 that was conducted for R1 for a higher level of care. Furthermore, the notice confirms that R1 would be transferred to a skilled nursing facility to meet R1’s needs. R1 confirmed that they were sent to a skilled nursing facility and are now residing at a wellness facility. LPA conducted an interview with two (2) staff that remember and knew R1. Let it be noted, R1 has not resided at the facility since December 2023. Both staff that were interviewed confirmed that R1 needed a higher level of care, the proper paperwork was conducted by the previous administration and R1 was transferred to a hospital under a 5150 diagnosis and then R1 was sent to a skilled nursing facility. LPA attempted to interview ten (10) residents regarding R1 but residents were not aware of who R1 was. Therefore, based on the interviews conducted and LPA's record review the allegation(s) is UNSUBSTANTIATED at this time.

An exit interview was conducted, no citation(s) were issue, and a copy of this report was given to the Administrator.
