Licensing Program Analyst (LPA) Tuesday Cabiness conducted the initial complaint visit to investigate the allegations mentioned above. LPA met with caregiver Shake Ekmekgian and informed her of the reason for the visit. Administrator Ani Akoubian was contacted by telephone and was also informed of the reason for the visit. The following information was obtained during the investigation.

Allegation #1: Staff locked a resident in their room. To investigate the allegation, on 09/10/2026, from 1:00 p.m. to 1:30 p.m., LPA interviewed the Reporting Party (RP) to obtain information pertaining to the allegation. During today’s visit, from 9:30 a.m. to 12:30 p.m., LPA interviewed the Administrator, caregiver, and two (2) of two (2) residents. LPA also obtained and reviewed resident and facility documentation.

The RP alleged that facility staff locked Resident #1 (R1) in R1’s room and kept indoors majority of the time. It revealed to LPA that R1 was admitted to the facility on 01/15/2026 and exhibited signs of mental health.....(Cont'd LIC9099C)

concerns. On 02/03/2026, facility staff contacted a mobile crisis safety team to evaluate R1. The RP was unable to provide a specific date or time frame when R1 was allegedly locked in the room.

LPA was unable to interview R1 regarding the allegation due to R1’s current location and a language barrier. The Administrator and caregiver denied that staff locked R1 in the room. Interviews with the two (2) current residents did not corroborate the allegation. Both residents were recently admitted to the facility and therefore did not have information regarding the alleged incident involving R1. Previous residents who may have had relevant information were unavailable for interview because they no longer reside at the facility. The two (2) current residents reported that they had never been locked in their rooms by facility staff. They also reported, being able to leave the facility as they pleased, and remain inside in the facility by choice. Based on the information obtained during the investigation: interviews, review of facility documentation, R1 unavailable for interview, the RP unable to provide a specific date or time frame for the alleged incident, and lack of witnesses, there is insufficient evidence to establish that facility staff locked R1 in the room. Therefore, the allegation is determined to be Unsubstantiated at this time.

Allegation #2: Staff threatened a resident. To investigate the allegation, on 09/10/2026, from 1:00 p.m. to 1:30 p.m., (LPA) interviewed the Reporting Party (RP) to obtain information pertaining to the allegation. During today’s visit, from 9:30 a.m. to 12:30 p.m., LPA interviewed the Administrator, caregiver, and two (2) of two (2) residents. LPA also obtained and reviewed resident and facility documentation.

It was alleged that staff threatened Resident #1 (R1) if R1 did not follow staff orders. The RP was unable to provide a specific date or time frame when R1 was allegedly threatened by staff.

Records reviewed by LPA indicated that R1 was admitted to the facility on 01/15/2026 and was reported to exhibit signs of mental health concerns. On 02/03/2026, facility staff contacted a mobile crisis safety team to evaluate R1. LPA was unable to interview R1 regarding the allegation due to R1’s current location and a language barrier. The Administrator and caregiver denied threatening R1. Interviews with the two (2) current residents did not corroborate the allegation. Both residents were recently admitted to the facility and therefore did not have information regarding the alleged incident involving R1.
