Licensing Program Analyst (LPA) Mariana Agban conducted an initial 10-day complaint visit to investigate the above allegation. LPA met with Staff#2 (S2) and the Assistant Administrator and Administrator Arman Voskerchyan were contacted and arrived shortly after. LPA explained the reason for the visit. At approximately 09:35 am, LPA conducted a physical plant tour. LPA requested copies of pertinent information which includes and not limited to Staff Roster, Staff files and Resident Roster. During today's visit, LPA interviewed three (3) residents and attempted for one (1) resident. In addition, LPA interviewed three (3) staff members.
Regarding the allegation: Administrator is not present at the facility a sufficient amount of hours to manage the facility.
It is alleged that Administrator comes to facility maybe once every three weeks and stays at the facility for ten minutes and leaves. LPA conducted interviews with three (3) residents. The residents consistently stated that the Administrator is not present at the facility for a sufficient amount of time to effectively manage its operations. The residents reported that the Administrator typically visits the facility once or twice a week, remains only a few minutes, and then departs. (Continue 9099C)

Interviews with staff revealed that the Administrator does not maintain a regular work schedule at the facility. Staff stated that the Administrator generally visits the facility two (2) to three (3) times per week, conducts a brief check on the residents, and then leaves shortly thereafter. Interview with the Administrator acknowledged that there is no established schedule for his presence at the facility. The Administrator stated that the length of each visit varies and may last approximately one (1) hour or longer, depending on the needs of the facility. However, the Administrator was unable to provide documentation, including the LIC 500 Personnel Report, or the Administrator's personnel file, as these records were not maintained at the facility.Based on interviews conducted and the lack of documentation demonstrating the Administrator's regular presence and oversight at the facility, the Department has sufficient evidence to support the allegation. Therefore, the allegation is deemed SUBSTANTIATED at this time. Per California Code of Regulations, Title 22, Division 6, Chapter 8, the facility is being cited on the attached LIC 9099D. An exit interview was conducted, a copy of this report is provided and Appeal Rights is also provided.
