Investigation revealed the following:

Allegation: “Staff does not ensure transportation vehicle is maintained in good working order”, it is being alleged that the facility’s bus lift (used for people with wheelchairs) is not in good repair.

Interviews conducted with S1 to S3 revealed the following: 3 out of 3 staff agreed with the allegation, moreover, staff indicated that the lift on the bus has not been operational for about a year.

Interviews conducted with R1 to R5 (residents who use wheelchairs) revealed the following: 3 out of 5 residents agreed with the allegation, furthermore, residents indicated that the bus lift has been non-operational for almost a year; 2 out of 5 residents were unable to provide additional information.

On 09/23/2026 at proximately 2:30 PM, the facility bus was toured. The facility bus was observed to be operational, but the driver indicated that the lift was in disrepair and not in use due to it being a safety hazard.

Record reviewed of Bus Repair Bids revealed the following: The facility has three bids/estimates for the facility bus lift.

Substantiated: Based on observation, interviews and record review, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, Title 22, Division 6 and Chapter 8 are being cited on the attached LIC 9099D.

An exit interview was conducted, appeal rights were reviewed, and plans of corrections were developed. A copy of this report was left with the Resident Service Director, MaryLou Escobedo.
