32 | Investigation revealed the following:
Allegation: Licensee did not ensure sufficient staffing to meet residents' care needs, resulting in multiple falls.
It is being alleged that the licensee did not sufficiently staff the facility, resulting in residents experiencing multiple falls.
On 09/24/2026, between 2:02 PM and 4:30 PM, the Department conducted an interview with Administrator Cristina Hale (S1). During the interview, S1 stated that the facility ensures sufficient staffing to meet residents’ care needs. S1 reported that the facility schedules adequate staff for each shift and, in cases of call outs or staff vacations, associates may pick up additional shifts. S1 also stated that the scheduling manager is trained to work the floor when needed. On 09/24/2026, between 2:02 PM and 4:30 PM the department asked S1 how many caregivers were scheduled and working on or around 09/04/2026, S1 stated that their best estimate was 6 to 7 caregivers in the morning shift and 5 to 6 caregivers in the evening shift. S1 was asked by the department if at any point that day only one caregiver was responsible for all residents, S1 said that such a situation has never happened. On 09/24/2026 the department asked how long any potential staffing shortage may have lasted on that date, S1 stated they could not recall and would need to review the staffing schedules. When asked whether any resident falls occurred on or around 09/04/2026, S1 stated they would need to review the incident reports to provide accurate information. S1 reported that the facility does not typically operate with staffing shortages. S1 stated that caregivers are not required to work overtime but may be asked when the need arises. When asked if they feel pressured to work additional shifts, S1 stated they do not feel pressured and typically work from 9 AM to 6 PM or until required work is completed. During the interview S1 was asked did the facility meet the advertised staffing ratios, of one caregiver for every 6 to 7 Residents. S1 said they would need to verify the facility’s materials but did not believe a 6–7 residents per caregiver ratio is advertised. S1 reported that the usual staffing pattern is 6 caregivers during the day shift, 5 caregivers in the evening shift, and 3 caregivers overnight, not including other staff. On 09/24/2026, between 2:02 PM and 4:30 PM the department asked S1 whether they have reported staffing concerns to management,
