32 | The investigation revealed the following:
Allegation: Staff did not provide adequate care resulting in resident developing infections
It is being alleged that R1 was placed in the above mentioned facility where he had developed several infections. On 7/6/26, LPA Felisa Shirley observed that R1 was not on the resident roster. LPA Shirley also reviewed annual records for this facility for 2025, 2024, 2023 and 2022 and noted that R1 was not listed as a resident. Upon review of Patient Service Instructions from St. Joseph Medical Supply, R1’s address was listed as an address from the prior facility that R1 resided. Per interview with S1 on 7/6/26, R1 was admitted to Bright Sunlife Guest Homes on 6/19/25 and resided at this facility for 1 day. LPA Shirley observed a text message dated 6/19/26 from a family member advising S1 that R1 would be picked up the next day at 1pm. Per interview with a family member on 4/28/26, the family arranged for R1 to be transferred to West Los Angeles Veteran Affairs Medical Center for further evaluation and care. On 7/6/26, LPA Shirley reviewed the discharge paperwork for R1 dated 5/22/25 from Hollywood Presbyterian Hospital and observed that R1 was treated for end stage kidney disease.
LPA interviewed staff 1 – staff 3 (S1 – S3). Of those interviewed 3 out of 3 denied the allegation. LPA interviewed resident 2 – resident 6 (R2 – R6). Of those who interviewed 4 out of 5 denied the allegation. One resident was not sure.
Based on information gathered, LPA did not find sufficient evidence to support the allegation “Staff did not provide adequate care resulting in resident developing infections,” therefore, the allegation is unsubstantiated.
No deficiencies were cited for these allegations.
An exit interview was conducted and a copy of this report was provided to the Caregiver, Algel Culala.
