32 | Allegation: Staff do not ensure that resident's incontinence needs are met.
Reporting Party (RP) alleges that staff do not provide R1 timely incontinence assistance.
LPA's interview with facility's Executive Director revealed the following: They refute the allegation, stating to LPA that R1 sustained an unwitnessed fall in early August 2026 and was sent to hospital for assessment/treatment. Upon R1's return to the facility, R1 receives intensive "Habilitation Hours" assistance from caregivers responding to residents on a 1-caregiver to 5-resident ratio. Executive Director stated that R1 is provided consistent and timely incontinence assistance by staff at least three (3) times daily, or more as needed. Staff check on R1 a minimum of every two hours to ensure R1's satisfactory health outcomes. Additionally facility has provided R1 with a walker, and wheelchair to enhance mobility, and that rehabilitation therapy (provided by Bonum Home Health) has recently begun for R1.
LPA's interviews with three (3) staff revealed the following: All staff refute the allegation, stating to LPA that R1 is provided timely and consistent incontinence assistance at least three (3) times daily, or more as needed.
LPA interviewed nine (9) out of ninety (90) residents, which revealed the following: All nine (9) out of nine (9) residents interviewed (including R1) stated they are satisfied with staff incontinence assistance and other ADL assistance and had no health/safety concerns.
LPA's records review of R1's file revealed the following: R1's physician's report (LIC 602) shows that R1 is challenged with schizophrenia, motor paralysis, and "refuses hygiene". Report shows that R1 lacks impulse control and exhibits aggressiveness by spreading feces on facility chairs. R1's service plan requires R1 to receive full staff assistance for bathing, dressing, toileting, and oral care.
During LPA's interview with R1, LPA observed the following: R1 was lying in his bed dressed appropriately and appeared comfortable. R1's room was clean and free of odor.
Based on LPA observations, and interview of records staff, and residents, there was not sufficient evidence to verify that staff are neglecting R1's needs for incontinence assistance. Therefore, the allegation is UNSUBSTANTIATED at this time. |
