Allegation: Staff do not provide adequate food service. It is alleged that on Friday, September 4, 2026 staff forgot to take dinner to the resident's room and as a result they did not eat dinner. It is also alleged that on Saturday, September 5, 2026, family sent the resident food, but staff took the food to the resident's room approximately 2 hours late. According to information obtained, facility staff's response was that they were short staffed that week. Record review confirms that families were notified on September 2, 2026, that the facility had a COVID outbreak that affected residents and staff. All staff denied the allegation. Staff interviews indicated that during the COVID outbreak residents that tested positive some residents were relocated to a designated isolation area, and others stayed in their rooms and received meal tray service in their rooms for during all meal times. A total of 13 residents were interviewed. None reported issues with meals being brought late to their room. There is insufficient information to support the allegation.

Allegation: Staff do not ensure residents’ showering needs are being met. The complaint alleges a resident had not been bathe for one week. All staff denied the allegation. They stated all residents are scheduled to be showered at minimum twice weekly, but during hot weather they are showered three times a week. According to staff interviews, even though some residents had COVID they were being showered, unless they were too weak or refused. Staff stated that when a resident refuses to be showered caregivers ask them a total of 3 times, then they follow "change of face" procedure, in which a different staff showers them, and if they continue to refuse showers it is communicated to the Resident Care Coordinator and/or med-tech. The AM shift caregivers shower residents between 6 AM- 2 PM, and afternoon shift showers residents between 3 PM - 9 PM. Two (2) out of 13 residents interviewed stated they were not showered as scheduled. The shower schedule was reviewed. During the week of August 31, 2026 - September 6, 2026 there were 15 staff that called off of work and some agency staff that covered shifts. According to staff interviews, despite call offs residents received scheduled shower assistance. All residents are on the shower list because even if they are independent, receive home health, or are in hospice caregivers are to check on the residents, to remind them to take a shower or to see if they need a shower. There is insufficient evidence to support the allegation.

Allegation: Staff do not ensure residents are assisted with dressing. It is alleged a resident's clothing was not changed in a week during the period of August 31, 2026 - September 6, 2026. All staff interviewed denied the allegation. They stated that residents are dressed before breakfast meal time and are not taken to eat breakfast in their pajamas. Bed bound residents are dressed before breakfast and eat their meals in their room. All residents denied the allegation. There is insufficient evidence to support the allegation.

Allegation: Staff did not ensure the facility was free from pests. It is alleged that cockroaches were observed in a resident's bathroom. A total of 13 residents were interviewed. Two (2) out of 13 residents stated they have seen cockroaches in their room. Staff interviews confirmed cockroaches have been observed in the kitchen and in some resident rooms. According to staff, there are residents that leave food out in their room, which may attract cockroaches. However, staff stated there have been no recent reports from residents regarding cockroaches. Administrator stated pest control is in place. Pest control service invoices were reviewed. Bi-weekly pest service visits are in place. A total of 23 rooms were inspected. No cockroaches were observed. Therefore, the allegation cannot be supported.

Allegation: Staff do not ensure residents room is cleaned properly. The complaint alleges that room floors are sticky and incontinence diapers have been observed in the resident's room floor, as well as fecal matter on the bathroom floor. A total of 27 rooms were inspected. Two (2) rooms had dirty and/or sticky floors and one (1) room had fecal matter on the toilet seat. Per staff interviews, rooms are deep cleaned once a week by housekeeping staff, room trash and incontinence briefs are to be removed by caregivers from the room every shift. Staff said there are several residents that rip off their incontinence brief and throw it on the floor, but during room checks if staff observe the brief on the floor it is immediately picked up. All staff stated that rooms are maintained and cleaned well by housekeepers. Staff stated that floors may be sticky due to too much cleaning disinfectant product used on the floor and because some residents spill drinks on the floor and do not report the spills. Based on physical plant observations, overall resident rooms are clean and maintained adequately. Therefore, there is insufficient evidence to support the allegation.

Allegation: Staff did not ensure residents' pendant was working properly. It is alleged that on September 3, 2026, a resident's wearable pendant alert device stopped working and staff took the pendant from the resident, but did not give the resident a new one until September 8, 2026. Instead the resident had to use their phone to call family so that they call the facility to notify them that their loved one needed assistance. Two (2) out of 13 residents stated that in the month of September 2026, staff took their pendants because they were not working and did not provide a new pendant. One resident stated staff took the pendant and 3 weeks later they still had not received a working pendant. Staff interviews revealed that Sage emergency call pendants automatically register and report low batteries and sends a signal to caregivers cell phone. The facility still has a working wall signal system, as well as Sage wall wall buttons. Staff stated that SAGE pendants work until their battery runs out, and some residents do not know understand the pendant has to be pressed. Additionally, there are some residents that decline pendant use. Based on physical plant observation, the pendants tested and wall signal system tests showed the alert system is operational. Therefore, there is insufficient evidence to support the allegation.

Allegation: Staff do not ensure resident’s incontinence needs are being met. The complaint alleges that due to the pendant disrepair a resident sat with a soiled incontinence brief until a staff checked on the resident. A total of 13 residents were interviewed, of which 3 stated staff sometimes take up to 30 minutes to assist with incontinence care after they press the pendant. Staff stated caregivers are to respond to toileting pendant calls right away or as soon as they can. Otherwise, incontinent residents are checked every 2 hours and during day time may be checked more often because residents drink more fluids and may need more incontinence assistance. Staff stated that to their knowledge their incontinence needs are met. There is insufficient evidence to support the allegation.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegations are Unsubstantiated.

An exit interview was conducted with Executive Director Colleen Rozatti. A copy of the report was issued.
