25 | On September 24, 2026, Licensing Program Analysts (LPAs) Ruth Martinez and Michael Tea conducted an unannounced visit and met with facility Administrator Virgilio Agas for the purpose of addressing the unlicensed care provided at this facility.
On July 3, 2026, a “Notice of Operation in Violation of Law” was served to prospective applicant for licensure Westminster Home Inc. following a modification of the administrative organization of the licensed entity. The operator is presently notified that all residents in care must be relocated within 15 days.
During today’s visit, LPAs toured the facility and observed no immediate health and safety concerns. Based on LPA’s interview with Administrator, review of facility’s roster, and observations, there are currently 51 residents who remain at the facility and are receiving elements of care and supervision.
The department received an application for a Change of Ownership; however, the application was denied on September 11, 2026. Consequently, the present “Notice of Operation in Violation of Law” is being served today. If operation of unlicensed care continues, civil penalties will be assessed starting October 4, 2026.
Based on this inspection, deficiencies were observed at this time in the areas evaluated per Title 22 Division 6 of the California Code of Regulations. See LIC 809-D for deficiencies.
This report was reviewed with Administrator Virgilio Agas, and a copy of this LIC809, LIC809-D report was provided and left at facility. Appeal rights reviewed, and a copy provided.
