25 | On October 15, 2026, Licensing Program Analysts (LPAs) Sean Haddad and Edward Kim conducted an unannounced Plan of Correction visit and met with Staff Rachel Chung for the purpose of addressing the unlicensed care provided at this facility.
On September 2, 2026, a “Notice of Operation in Violation of Law” was served following a substantiated investigation regarding unlicensed care at this facility. The operator was notified that all residents in care must be relocated within 15 days.
The department received an application for a Change of Ownership; however, the application was denied on September 11, 2026. Consequently, a second “Notice of Operation in Violation of Law” was served on September 24, 2026. The operator was notified that, due to continued operation without a valid license, civil penalties would be assessed commencing October 5, 2026.
On October 6, 2026, civil penalties of $200 per resident, per day, were assessed for the 50 residents in care observed for the one-day period of October 5, 2026, in the total amount $10,000.
During today’s visit, LPAs toured the facility and observed no immediate health and safety concerns. Based on LPA’s interview with staff, review of facility’s roster, and observations, there are currently 56 residents who remain at the facility and are receiving elements of care and supervision as identified on the LIC811. Civil penalties of $200 per resident, per day, are being assessed for the 56 residents in care for the ten-day period starting October 6, 2026, to and including October 15, 2026, in the total amount $112,000.00.
An exit interview was conducted, and a copy of this report, LIC421A, LIC811, and appeal rights were provided to Staff Rachel Chung on behalf of operator Shin Kang. |
