13 | Licensing Program Analyst (LPA) Melissa Parks arrived unannounced on Thursday September 10, 2026, to complete and deliver findings to a complaint received on July 16, 2026. LPA met with Administrator Megan and explained purpose of visit.
LPA learned the following: R1 has a routine order for Lorazepam to be given once daily. According to R1’s MARs, this is scheduled to be given at 8am. On 7/13/2026, Maybeth, the facility’s Memory Care Director, observed the narcotic count sheet for R1’s Lorazepam to be incorrect. Maybeth then locked the narcotics in her office. When S1 began their shift on 7/14/2026, R1’s medication was not in the med cart. S1 contacted Maybeth regarding the medication. Maybeth instructed S1 to obtain her office keys from the front desk and obtain the medication. S1 declined. According to facility lobby cameras, Maybeth arrived at work on 7/14/2026 at 8:51am. According to Maybeth, she then gave R1 their medication and documented on the narcotic count sheet and eMAR.
