On 8/20/2026, Licensing Program Analyst (LPA) Mary Rico conducted an unannounced visit investigate and deliver findings on the allegations listed above. LPA met with staff and explained the purpose of the visit. The investigation consisted of staff interviews, resident interviews and record reviews. The Administrator Ducle Redford was contact and informed about today's visit.

For the allegation, Staff is withholding resident’s medication. During staff interviews two out of the two staff stated they are not withholding any resident’s medication. During resident interview two out of the two residents stated their medication is not being withhold. During medication audit, LPA observed staff are not withholding residents medications.

For the allegation, Staff is not meeting residents needs. During staff interviews two out of the two staff stated they meet all residents’ needs. During resident interviews, two out of the two residents stated their needs are being met.

For the allegation, Unlawful eviction. During staff interviews two out of the two staff stated that no eviction was provided to R1. During resident interviews, R1 stated no eviction was provided.

Based on the evidence found during the investigation, three (3) allegation listed above are deemed UNSUBSTANTIATED. A finding that the complaint are UNSUBSTANTIATED means although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and this report (LIC9099) was discussed and provided to Administrator Dulce Redford.
