Licensing Program Analyst (LPA) Andrew Martinez made an unannounced visit to the facility for the purpose of completing an investigation into the above complaint allegations. LPA met with Executive Director, Andrea Perez, and explained the reason for the visit. Today’s visit consisted of observation, staff and resident interviews, records review and obtaining pertinent documents relating to the investigation.

For the allegation: Staff do not maintain facility free from hazards. Based on staff interviews, six (6) out of six (6) staff members stated no health hazards have been reported to them including any asbestos problems, and three (3) out of six (6) staff members stated the building has had no history of being treated for an asbestos issue, to their knowledge. Based on resident interviews, five (5) out of six (6) residents stated there were no asbestos issues in the building, and one (1) out of (6) residents stated they did not know if there was or not. Based on LPA’s observation of facility's indoor premises (e.g., basement, first floor, second floor, third floor), no immediate health and safety concerns were visible, and no reliable visual identification of asbestos fibers could be identified. Based on interviews and observation, this allegation is UNSUBSTANTIATED. ***Continued to LIC 9099-C***
