Licensing Program Analyst (LPA) Andrew Martinez made an unannounced visit to the facility for the purpose of completing an investigation into the above complaint allegations. LPA met with Executive Director, Andrea Perez, and explained the reason for the visit. Today’s visit consisted of observation, staff and resident interviews, records review and obtaining pertinent documents relating to the investigation.

For the allegation: Insufficient food services. Based on LPA's observation, the facility has an ample supply of food for residents in care with extra supply of prepared items during lunchtime meal service. Based on staff interviews, five (5) out of six (6) staff stated there are no issues with having enough food in stock for residents. Three (3) out of six (6) staff stated the kitchen may run out of a listed menu item on the day it is being served during a particular meal service for residents who may arrive at the dining room toward the end of meal service, however, other food options are always available to serve. Based on resident interviews, three (3) out of five (5) residents stated the food service is good, they like the food, and the facility doesn't run out of food. One (1) out of (6) residents stated the kitchen often runs out of food items during dining hours and one (1) out of six (6) residents stated staff will say they're out of an item then see it being served to another resident. Based on observations and interviews, this allegation is UNSUBSTANTIATED. ***Continued to LIC9099-C***

For the allegation: Licensee does not ensure that the facility is kept free from pests. Based on staff interviews, five (5) out of six (6) staff stated there have been no complaints about pests made to them and they have not witnessed an issue with pests. Three (3) out of six (6) staff stated an exterminator comes on a regular basis for the maintenance of pest control. Based on resident interviews, four (4) out of five (5) residents stated there have been no issues with pests in the facility. One (1) out of five (5) residents stated they saw two roaches in the dining room, reported it to staff and had not seen any other pest problem since then. LPA observed that the facility maintains a Customer Pest Sighting Log with approximately three entries on 01/13/26, 02/23/26, and 04/10/26, indicating a resident has mentioned the sighting of a possible roach in their rooms. LPA observed semi-monthly pest & rodent control service invoices from Western Exterminator Company completed on 11/01/25, 12/01/25, 01/01/26, 02/01/26, and 04/01/26, showing the staff have a pest control maintenance plan in effect. Staff #1 (S1) stated they can also call for service as needed yet have not had to do so. Based on observations and interviews, this allegation is UNSUBSTANTIATED.

For the allegation: Licensee does not ensure that residents are provided with activities. Based on staff interviews, five (5) out of (5) staff stated the facility provides monthly activities calendars to all residents for the upcoming month's planned activities and outings, as well as a daily schedule of activities printed out and posted in various locations throughout the facility for visibility to all residents and guests. Two (2) out of five (5) staff stated participation in the facility’s activities is encouraged and plans are in place to integrate new residents with familiarity with and participation in planned activities. The facility employees an Activities Director and Assistant Activities Director who ensure all staff and departments are supplied with activities calendars to maintain awareness of and knowledge of the upcoming activities and outings. LPA observed multiple postings of monthly, special weekly, and daily activity calendars posted in the halls, dining room, activities rooms and spaces, and on each floor of the facility. Based on resident interviews, five (5) out of five (5) stated there are plenty of activities available to residents to participate in. Based on observations and interviews, this allegation is UNSUBSTANTIATED.

Based on the evidence, the allegations mentioned above are UNSUBSTANTIATED. A finding that the complaint is UNSUBSTANTIATED means although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur.

An exit interview was conducted where this Complaint Investigation Report (LIC 9099 and LIC 9099-C) was discussed with and copies provided to Executive Director, Andrea Perez.
