13 | On June 30, 2026, at 9:00 AM, Licensing Program Analyst (LPA) Avelina Martinez arrived at the facility unannounced to deliver complaint findings. LPA Martinez met with Kathryn Nevin during today’s visit.
Throughout the course of this investigation, The Community Care Licensing Investigation Branch conducted interviews, reviewed facility records, and resident records.
Based on the complaint findings, it was learned that facility sought out medical attention for resident 1 (R1) in a timely manner. In addition, R1's death certificate indicated cause of death was not questionable.
Due to the above noted information, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, and therefore the allegation is unsubstantiated. An exit interview was conducted, and a copy of this report was provided to the facility.
