25 | On July 09, 2026, at 9:30 AM, Licensing Program Analysts (LPAs) Avelina Martinez and Sulma Lopez made an unannounced visit to this facility to conduct an annual required inspection. LPAs met with melda Pratt and Ignacio Illana and explained the purpose of today's visit.
LPAs inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, activity room, and outside courtyards of the facility to ensure compliance with Title 22 regulations.
Administrator holds current certificate. The facility is licensed for six non-ambulatory residents. There are currently five residents who reside at this facility. The facility has an approved hospice waiver for six.
The LPAs toured the facility with Ignacio Illana on July 09, 2026, at 10:45 AM.
LPAs reviewed five resident files and two staff files. All files were maintained and kept current. The facility has a natural disaster plan and an infection control plan. The last emergency drill was on April 17, 2026. LPAs reviewed two medication administration records (MAR).
The facility temperature measured at 73 degrees, and the facility water temperature measured at 115 degrees. The facility fire extinguishers, smoke detectors, and carbon detectors were in good repair. The facility common rooms were furnished and in good repair. The facility has a public telephone. The facility has adequate food supply. The exterior of the home has a shaded patio for resident use, and the emergency exit gate is in good repair.
Based on this annual inspection, there were no deficiencies. An exit interview was conducted, and a copy of this report was provided to the facility
