On September 16, 2026, at 2:00 PM, Licensing Program Analyst (LPA) Sulma Lopez arrived unannounced at the facility to deliver complaint findings. LPA Lopez met with Administrator Glenda Molinyawe and explained the purpose of the visit. The current census was 6 residents.

On June 29, 2026, the Department received a complaint allegation about the facility staff overcharging for medical appointments.

Throughout the course of the investigation, LPA Lopez toured the facility, conducted interviews, and record reviews. Based on interviews with residents, it was learned that residents do not leave the facility for medical appointments. Resident 1 (R1) stated that they do not utilize transportation for medical appointments as they have telehealth appointments. Interviews with Resident 2 (R2) revealed that they do not utilize the care home's transportation services, but rather they utilize Paratransit if they ever need transportation. Continued...

When residents were asked if they had ever been charged for transportation by the facility, they stated they had not.

Interviews with staff revealed that transportation services are offered for an additional fee if needed. Administrator (A1) explained that transportation services have not been utilized by any of the residents as they all receive virtual or home health services.

A tour of the facility on multiple occasions revealed that residents do not leave the facility or utilize the care home's transportation. LPA observed families visiting residents in the home. LPA also observed home health nurses providing care in the facility, and LPA observed that residents remained in the home during 3 out of 3 LPA visits in the past 2 months.

Record reviews of the residents' admission agreements revealed that the facility does not provide transportation services to the residents. The agreement states "We do not provide but will assist in making available to residents transportation to the nearest appropriate health facilities for medical and dental appointments, social services agencies, shopping and recreational facilities, and religious activities."

As a result of the investigation, the Department finds this allegation to be Unfounded. A finding that the complaint is unfounded means that the allegation was false, could not have happened and/or is without a reasonable basis. Per California Code of Regulations (CCR) - Title 22, no deficiencies were cited during this visit.

An exit interview was conducted with, and a copy of this report was provided to the facility.
