32 | On January 09, 2026, LPA Valerio conducted an unannounced visit to the facility. LPA Valerio observed two staff, Staff 1 and Staff 2, on shift. LPA Valerio observed one staff to be without a fingerprint clearance and the second staff without a facility association through Guardian. LPA Valerio cited Licensee/Administrator Beatrice Clark on case management visit dated January 09,2026.
An interview was conducted with Staff 1 (S1) and Staff 2 (S2) on January 09, 2026. According to (S1), S1 reported that S1 is a new staff member and works at night time. S1 stated S1 works 6:00 PM to 6:00 AM. According to an interview with S2, S2 works 6:00 AM to 6:00 PM. S2 stated S2 will leave when S2 feels comfortable leaving the residents. S2 reported that S2 will sometimes leave at 8:00 PM.
According to an interview with Administrator Beatrice Clark, the staff on shift only always work right hours. However, based on interviews with staff on January 09,2026, staff stated they worked 6:00 AM to 6:00 PM. When asked if Administrator Beatrice keeps track of staff hours, she stated, "No, I do not. They just text me their hours. I pay them overtime." Administrator stated she works at the facility three to four days straight and sometimes Staff 3 works in the evening.
LPA Valerio conducted interviews with residents, Resident 1 (R1) - Resident 4 (R4). Interviews with R1 and R2 were deemed unsuccessful due to communication barriers. According to an interview R3, R3 stated they had a call button before but not anymore. R3 reported that R3 calls for staff and staff come to help. R3 stated that in the middle of the night, R3 will call for staff and they come right away. R3 confirmed that two staff, Staff 1 and Staff 2 assist R3. According to an interview with R4, R4 reported that if R4 needs assistance R4 will call out for help. R4 stated they need more than just one staff on shift. The one staff that is here works 24/7 and simply cannot do everything all the time.
Based on all the information collected by the Department, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation occurred, therefore this allegation is UNSUBSTANTIATED. California Code of Regulations (CCRs) - Title 22, Division 6, Chapter 8, no deficiencies cited. An exit interview was held with facility staff, and a copy of report was left at the facility. LPA Valerio informed Administrator Beatrice of findings via cell phone. Administrator Beatrice gave permission for staff on shift to sign on her behalf. |
