32 | Allegation: Staff did not afford a resident privacy.
LPA interviewed residents, residents stated that are given privacy for personal visits and when requested. Staff interviews revealed they give residents privacy when requested as well as privacy with visitors.
Allegation: Staff did not abide by the admission agreement.
LPA reviewed Resident #1 (R1) admission agreement. Per R1s admission agreement it was agreed upon what R1 would pay every month. Both R1 and Licensee signed the admission agreement on 06/15/2026. There were no changes to the admission agreement at the time of move in.
Allegation: Staff are not properly trained on how to handle a resident.
Based on Resident #1 (R1)’s physical therapy plan, R1 required a Hoyer lift for assistance. Staff interviews revealed they were trained by physical therapy and the facility nurse multiple times on how to use the Hoyer lift. Staff would use the lift to assist with moving R1. Based on documentation and interviews conducted, there were no incidents with facility staff using the Hoyer lift with R1.
Based on information obtained through interviews, the Department finds the allegation to be UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis.
Exit interview conducted and a copy of the report and appeal rights were left at the facility. |
