Licensing Program Analyst (LPA) Talwinder Bains arrived on 9/9/26 to conduct the annual inspection.
LPA met with House Manager , Ida Briones who assisted LPA during today's inspection.

From gathered information, it was found out that resident, R1 never reside at this facility since it was operational so this allegation was UNFOUNDED.

A finding that the allegation is unfounded means that the allegation is false, could not have happened, and/or is without a reasonable basis.

Exit interview was conducted with Ida. A copy of this report was provided.
