13 | On 8/19/2026, Licensing Program Analyst (LPA) Eldin Serrano made an unannounced visit to the facility to commence the complaint investigation and deliver the findings of the above allegations. LPA explained the purpose of the visit to Sales Director Lorena Munoz. The investigation consisted of records review, interviews with the reporting party, relevant parties, staff, and residents, as well as facility observation.
Allegation #1 Resident sustained bruises due to neglect or physical abuse
Based on all the interviews, as well as LPA observation, the evidence does not support the allegation. RP stated R1 had three small dots on R1’s wrist but could not identify the cause. LPA observed a minor scratch on R1’s wrist, not a bruise. All staff interviewed denied witnessing physical abuse, and residents denied observing abuse. Interviewed with relevant party reported that R1 has a long history of prior unsubstantiated complaints. No documentation or witness statements corroborated with physical abuse or neglect. LPA is unable to corroborate the allegation.
