On 3/28/2022, LPA Hilario reviewed and collected resident and staff records. LPA conducted a tour of the facility and observed residents in care. Further investigation is needed, and future visits may be necessary to determine the outcome of the investigation.

On, 10/4/2023, 10/6/2023 & 7/10/2024, LPA Serrano interviewed staff and obtained facility records. More time is needed to reach a determination for the above allegations, therefore, this Complaint needs further investigation.

For the allegation resident sustained injury while in care. LPA attempted to collect records from facility but according to Health Services Director Jenalyn Arandia searched and confirmed that there are no files for R1.

Facility license # 374603394 has been closed since 9/30/2024 due to a change in ownership.

Based on interviews, the department has determined that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
