13 | Licensing Program Analyst (LPA) Iby Strong sent this report to the former licensee's last known mailing address via USPS certified mail to deliver the investigation findings for the above-mentioned allegations. This facility ceased operations on April 7, 2026.
In November of 2021, Community Care Licensing (CCL) received a complaint alleging Resident 1 sustained multiple injuries due to lack of supervision and R1’s needs were not met. During the investigation the Department collected records and conducted interviews. According to the allegations, facility staff were not preventing R1 from falling therefore R1’s needs were not met as R1 was known to be a fall risk. Interviews and records revealed that R1 had a major neurocognitive disorder and was non-ambulatory. Interview with an outside source established that R1 would forget they could no longer walk but would still attempt to transfer resulting in falls. There was no additional information available to corroborate that the facility did not provide supervision or fulfil the needs of the residents in care.
Based available information there was not a preponderance of evidence to prove the alleged violations occurred, therefore the allegations are unsubstantiated. A copy of this report was mailed to Licensee via USPS Certified Mail.
