13 | Licensing Program Analyst (LPA) Iby Strong sent this report to the former licensee's last known mailing address via USPS certified mail to deliver the investigation findings for the above-mentioned allegations. This facility ceased operations on April 7, 2026.
In December of 2023, Community Care Licensing (CCL) received a complaint alleging staff did not properly assist residents causing a fall and subsequent injury, staff did not report R1’s fall to responsible party, staff did not transfer R1 properly after the injury and R1 was handled roughly. During the investigation the Department attempted to collect records and conducted interviews. Interview conducted with an outside source who is close to R1 established that there were no issues with the care R1 received while at the facility. According to outside source, facility provided care that went above requirements. Interview with staff did not confirm any of the allegations. There was no additional information collected that corroborated the allegations mentioned.
Based available information there was not a preponderance of evidence to prove the alleged violations occurred, therefore the allegations are unsubstantiated. A copy of this report was mailed to Licensee via USPS Certified Mail.
