Interviews with R1 said that they did not say that they did not report that they disliked the food at the current facility they are living at. They had reported to a third party that their former facility would not provide them with hot food when they returned from their appointments. R1 also reported that at the current facility, although they provide them with some of the same foods, they liked that they are receiving their meals hot. The facility obtained internal records of the residents' meals that were provided to them. Upon review, the resident was being provided with oatmeal, berries, pancakes, sausage, and fruit. During the visit, LPA observed that staff were cooking, and upon R1's return from their appointment, they were provided with pancakes, sausage, and fruit served with a cup of coffee.
It was specifically alleged that the resident was missing 5 pairs of pants and towels. According to facility staff #1 (S1), they said that they had inventoried the resident's belongings when they were transported to the facility. S1 said R1 was an emergency transport and their belongings were transported by a third-party agency when they arrived. They had inventoried their items upon their arrival and said that they did not have any additional pants beyond the ones inventoried. R1 said that they count their items daily and they had counted 10 pants, and later recanted and said 11 due to the pair that they wore. R1 said that they counted the items when they were on their bed when they arrived at the facility and put them away themselves in their drawer. LPA counted the pants with R1 and with S1. There were a total of 5 pairs, including the pair that R1 wore. They also said that they were missing towels, and when LPA commenced to count the towels with R1, they said that there were towels in their drawer that were not there the night before, and some of the towels they felt were not theirs. Upon LPA's entrance, they had counted with S1 a total of 13 towels. According to the resident's Personal Property and Valuables Sheet, the resident arrived at the facility with 6 pairs of pants and a total of 13 towels. LPA spoke with the Administrator, who said that they had received R1's items in garbage bags from their third-party agency and had inventoried the items upon arrival. The Administrator also said that they had been informed that R1 was missing 5 pairs of pants and had gone out to purchase the missing items and towels to de-escalate the situation because R1 was upset.
It was specifically alleged that staff were mismanaging R1s medications, and the counts of the medications are not always the same. Interviews with R1 said that the staff are giving them incomplete medication and counts between 12 -13 medications, and now they have been counting between 7 or 6 medications.
