32 | On 04/08/2026 and 06/02/2026, LPA interviewed 3 staff (S1, S3, S4) and 5 residents (R1, R2, R3, R4, and R5) regarding the allegation. S1 stated staff became aware of the resident conflict, S2 responded, the administrator and law enforcement were notified, and the facility implemented resident warning notices and safety plans. S3 and S4 stated staff routinely monitored residents, redirected verbal conflicts, and separated residents when necessary. Neither S3 nor S4 witnessed the reported incident. R2 stated R1 verbally abused and threatened R3 and stated staff contacted law enforcement following the incident. R1 and R3 stated staff intervened during the incident. R4 stated staff respond when residents argue and direct residents to stop arguing and return to their rooms. R5 did not provide information regarding the reported incident or staff.
On 06/02/2026, LPA attempted to interview staff 2 (S2); however, S2 was unavailable and out of the country. On 07/03/2026, LPA attempted to contact S2 by telephone and left a voicemail requesting a return call. No response was received. LPA reviewed S2's written statement, which documented staff responding after becoming aware of the resident conflict, provided verbal redirection, intervened during the incident, and continued monitoring the involved residents.
LPA reviewed facility surveillance video of the incident and observed S2 responding and verbally redirecting a resident and return to intervene after the resident proceeded toward R1s room the second time.
LPA reviewed resident records, incident documentation, progress notes, administrative review, safety plans, resident warning notices, and staff training records. Records reviewed documented resident care needs, supervision requirements, behavioral interventions, staff intervention, increased monitoring, follow-up administrative actions, and completion of annual RCFE training related to resident care and supervision.
Based on interviews conducted, observations made, and records reviewed, staff responded after becoming aware of the reported resident conflict, intervened during the incident, and implemented resident warning notices, increased monitoring, and safety plans following the incident. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur; therefore, the allegation is unsubstantiated.
No deficiencies were cited based on California Code of Regulations (CCR) Title 22. An exit interview was conducted with Administrator Nicholas Inneh and a copy of the report was provided.
