13 | On 5/22/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to open this complaint. LPA Calandra was greeted by Stephanie Brice, Executive Director and explained the purpose of the visit.
Complaint alleges that facility staff financially abused a resident in care. According to the reporting party, the victim is a resident at the facility and reported fraud on their account(s). Reporting Party stated that R1 reported to them that R1's passport and credit cards were stolen by a staff member(S1). Based on interviews and record review, R1 reported fraud/financial abuse but no evidence is present.
LPA collected the following documents during the investigation: LIC 602(Physician's/Medical report), recent appraisals of Needs and Services, and staff schedules.
Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
No deficiencies cited during today's visit. An exit interview was conducted and a copy of the report provided to facility representatives. |
