Regarding allegation “Licensee did not provide a copy of the admission agreement to resident's responsible person”. It was alleged by the Reporting Party (RP) that upon Resident #1’s admission to the facility, on 07/8/2026, a copy of the Admission Agreement was not provided despite multiple requests for the document.
During the investigation, the Licensee acknowledged that as of today, a copy of the Admission Agreement had not been provided to the RP. The Licensee stated that it was their intention to provide a copy; however, they explained that the Admission Agreements are in spiral-bound notebooks, and copies are not readily available. The Licensee explained that the documents would need to be removed from the binding in order to make copies. Based on the information obtained during the investigation, the licensee did not provide a copy of the signed and dated current Admission Agreement immediately upon signing it. LPA determined there is sufficient evidence to substantiate this allegation.
Pursuant to Title 22, California Code of Regulations and/or CA Health and Safety Code, the following deficiencies were cited (refer to LIC 9099-D.) Administrator was informed that failure to correct the deficiencies may result in civil penalties.
Exit interview conducted, appeal rights discussed, and a copy of this report and appeal rights were provided.
